Treatment of severe acute pancreatitis by using early microtraumatic perioreal lavage

Abstract

Objective To investigate the clinical effect of peritoneal lavage through placement of central venous catheter in the treatment of patients with severe acute pancreatitis(SAP).Methods 46 patients with SAP received peritoneal lavage were retrospectively analyzed.Conservative treatment were allowed.Ascites amylase,APACHE Ⅱscore,serum amylase activity and plasma concentrations of TNF-α and IL-8 were measured before peritoneal lavage and 24,48 and 72 hours after peritoned lavage.Results All patients′ general condition improved to some extent after treatment.The ascites amylase significantly decreased from(2 057.6±91.7)u/L to(261.6±26.9)u/L,and the APACHE Ⅱ score decreased significantly from 13.1±2.9 to 4.3 ±1.4(P0.01).Serum amylase activity,plasma concentrations of TNF-α and IL-8 were decreased from(3 775.4±447.2)u/L,(1 187.6±203.6)ng/L,(345.3±72.4)ng/L to(988.6±201.7)u/L,(415.9±67.4)ng/L,(194.1±60.5)ng/L(P0.01)respectively.Conclusion Peritoneal lavage through placement of central venous catheter is feasible and micro-invasive with encouraging results.It is an effective and safe treatment option for patients with SAP.
