Abstract
Objective To investigate the correlation of elevated serum cardiac biomarkers and cardiovascular diseases of emergency patients in cardiovascular hospital. Methods Serum concentrations of cardiac troponin I (cTnI) and Creatine kinase MB mass (CK-MB mass)in 155 emergency patients with cardiovascular diseases were measured by chemiluminescent immunoassay(CLIA).The patients were divided into three groups according to clinical diagnosis:acute myocardial infarction group (AMI group),unstable angina group (UA group) and other diseases group.Comparing the variation of cTnI and CK-MB mass among the three groups. Results In 155 patients,AMI group had 114 cases,UA group had 22 cases and other diseases group had 19 cases (12.3%).The level of serum cTnI and CK-MB mass of AMI patients were significantly higher than that of the UA patients and other diseases patients, while there were no differences between the UA patients and other diseases patients. The cTnI level showed a significant positively correlation with CK-MB mass (r=0.608,P=0.000) in 155 patients. In other diseases group, the level of serum cTnI concentrations of aortic dissection (AD) and dilated eardiomyopathy (DCM) patients elevated significantly, they can achieve 10 times the cut-off value of AMI.Conclusion The serum cTnI and CK-MB mass were nearly correlated with the degree of myocardial damage. The level of serum cTnI showed significant positively correlated with CK-MB mass, and correlation was nearly. To diagnose AMI,serum cTnI was preferred,cTnI and CK-MB mass need not be detected at same time.In patients with cTnI0.5 ng/ml,in addition to ACS,physicians should consider AD and DCM.In order to improve the diagnosis of AMI,we suggest that combining cTnI and myoglobin (Mb) or combining CK-MB mass and Mb.
