Clinical significance of urinary type IV collagen for senile early diabetic nephropathy

Abstract

? Objective To clarify the diagnostic relevance of urinary typeⅣ collagen(CⅣ) in senile diabetic nephropathy(DN).Methods Urinary CⅣ was determined by radioimmunoassay in patients with type 2 diabetic mellitus patients,42 cases of normalabuminuria(A group), 54 microalbuminuria(B group),17 cases of macroalbuminuria(C group) and 38 normal subjects as control.Results The mean concentration of urinary CⅣ in total 113 diabetic patients〔(124 99±52 93) μg/24h〕 was higher than that of normal control〔(82 18±12 98) μg/24h〕(( P 0 01), the unrinary CⅣ was increased even if type 2 diabetic patients with normalalbuminuria,Group A 〔(94 24±25 43) μg/24h〕.Group B〔(117 32±32 02) μg/24h〕 and group C〔(225 33±34 55) μg/24h〕 were remarkabely higher as comparable with control( P 0 05, P 0 01, P 0 001).There was significantly positive correlation between urinary CⅣ level urinary albumin excretion(UAE),α 1 macroglobulin(α 1 MG),N acetyl β D glucosaminidase(NAG),SBP,FBG,HbA 1c.Conclusions Our results suggest that measurement of the urinary level of CⅣ may have potential for the evaluation of senile early DN. 〔
