Analysis of left ventricular dsyfunction in acute ST segment elevation anterior myocardial infarction after primary percutaneous coronary intervention

Abstract
Objective:To analyse left ventricular dysfunction for patients of acute ST segments elevation anterior myocardial infarction (STEMI) after primary percutaneous coronary intervention (PCI) with Thrombolysis in Mycardial Infarction (TIMI) 3 grade.Method:The total of 112 patients after primary PCI were analysed between January 2004 and January 2009.Fifty-two patients of left ventricular dysfunction in acute anterior STEMI with TIMI 3 grade after primary PCI were divided treated as disease group.Sixty patients from randomized other of normal ventricular function in acute anterior STEMI with TIMI 3 flow grade after primary PCI were treated as control group.Data was analyzed with Multiple logistic regression analysis.Result:Decrement of WMS,decrease of left ventricular ejection function (LVEF) and cardiac index (CI),and enlargement of left ventricular end-diastolic diameter (LVED) and left ventricular end-diastolic volume (LVEDV) were found in the patients with left ventricular dysfunction disease group after firt day and two weeks with primary PCI compared with those in control group(P0.01).LVEF,CI,and WMS improved in patienrs of KillipⅡ class after two weeks (P0.01),then,LVEF,CI decreased,LVED,LVEDV and LVESV enlarged in patienrs of KillipⅢ and Ⅳ class after two weeks with primary PCI (P0.05).Pre-infarction agina,and thrombolysis in mycardial infarction myocardial blush grade (TMBG),wall motion score (WMS) are the relative factors of lefr ventricular dysfunction (P=0.04).Conclusion:Left ventricular dysfunction of the patients in acute antetior STEMI,who were successfully performed by PCI with TIMI 3 grade may be related to microvascular injury of ischemic myocardium,systolic and diastolic function decreased,and left ventricular enlarged,and LVEF was improved by left ventricular enlargement.
