Abstract

Recent years, a new hemodynamic monitoring method that pulse indicator continous cardiac output (PiCCO) technique was applied to the clinical by Europe-America scholar. The PiCCO method was a combination of single transpulmonary thermodilution technique and arterial pulse count analysis. The single transpulmonary thermodilution technique requires a central venous line and an arterial catheter placed in one of the bigger system arteries to determine thermodilution cardiac output(CO), intrathoracic blood volume(ITBV) and extravascular lung water(EVLW). Neumerous reports have demonstrated ITBV to be able to reflect the intravascular volume status and to assessment the cardiac preload more adequate and more sensitive than CVP, PAOP or RVEDP. This article reviewed cardiac preload definiton from the cardiovascular physiology and interferential factors from the pathophysiology and clinical application, in the meanwhile the mechanism and the clinical reports point to be such as above pressure measure unreflect cardiac preload were enumerated. It was detail explained that ITBV's fundamental principle, mathemetics, correct formulas and attentions. In view of the present some contrast results for compare ITBV with PAOP e.g.cardiac filling pressure rather than ITBV should be used to monitoring fluid administration in moderately volume-depleted patient groups with and without cardiac disease. So that author suggest clinician even if applying PiCCO technique to firmly regard ITBV as cardiac preload, still should be attention filling pressure monitor.
