OBJECTIVE:To analyze the risk factors of cervical carcinoma with para-aortic lymph node metastasis in order to provide the basis for individuals therapy for the patients with cervical carcinoma.METHODS: The risk factors of 298 patients with cervical carcinoma receiving radical hysterectomy and pelvic lymphadencetomy and paraortic lymphadenectomy were analyzed retrospectively.RESULTS:There were 6 691 lymph node dissected in 298 patients,including 99(326 lymph node) with lymph node metastasis.Among these patients,26(8.7%) had para-aortic lymph node metastasis,33 had positive common iliac lymph node,and 97 with pelvic lymph node metastasis.The single paraaortic lymph node metastasis was in 2 cases,and 20 cases combined with common iliac lymph node metastasis.Univariate analysis showed that vessel invasion(χ2=4.198,P=0.040),serum level squamous cell carcinoma antique(SCC-Ag)4 μg/L before treatment(χ2=4.452,P=0.035),common iliac lymph metastasis(χ2=92.942,P=0.000) and number of pelvic lymph metastasis(χ2=51.230,P=0.000) were associated with para-aortic lymph node metastasis.Factors prediction of para-aortic lymph node metastasis on Logistic forward regression were common iliac lymph metastasis(P=0.000) and number of pelvic lymph metastasis(P=0.001).CONCLUSION:Cervical carcinoma with para-aortic lymph node metastasis is related to the number of Common iliac and pelvic lymph node metastasis,patients who with these two risk factors may suffer the risk of para-aortic lymph node metastasis,so they need to be evaluated seriously and formulated the individualized treatment.
