Abstract

Objective:To evaluate the relationships between admission fasting plasma glucose(FPG) concentration and cardiac function status in chronic congestive heart failure(CHF) patients with or without diabetes.Method:From July 2009 to September 2010,we investigated 161 hospitalized CHF patients with or without diabetes that the cardiac function status was grade Ⅱ to grade Ⅳ according to the cardiac function standard of New York Heart Association(NYHA).The 161 patients with or without diabetes were divided into 4 groups by the admission FPG: 6.1 mmol/L group(n=56),6.1 to 7.7 mmol/L group(n=41),7.8 to 11.0 mmol/L group(n=38) and ≥11.1 mmol/L group(n=26).The relationships between the FPG concentration and cardiac function status in patients with or without diabetes were analyzed.Result:①Compared to the paitents in FPG 6.1 mmol/L group,the proportion of patients with diabetes in 6.1 to 7.7 mmol/L group,7.8 to 11.0 mmol/L group and ≥11.1 mmol/L group were significantly higher(P0.01),the body mass index(BMI),the concentration of total cholesterol(TC) and low density lipoprotein cholesterol(LDL-C) were also significantly higher(P0.05) and the left ventricular ejection fraction(LVEF) were significantly lower(P0.05);the proportion of patients with NYHA cardiac function status grade Ⅳ in 6.1 to 7.7 mmol/L group,7.8 to 11.0 mmol/L group and ≥11.1 mmol/L group were significantly higher than those in FPG6.1 mmol/L group(P0.05).②After adjusting age,sex,BMI,blood pressure(BP),heart rate(HR) and serum lipid with covariance analysis,the LVEF of the paitents without diabetes in FPG 6.1 mmol/L group,6.1 to 7.7 mmol/L group and 7.8 to 11.0 mmol/L were significantly higher than that of the paitents with diabetes(P0.05),but significantly lower in FPG ≥11.1 mmol/L group(P0.05).③Multiple linear regression analysis showed that adjusting age,BMI,BP,HR andserum lipid,in the whole paitents,in the diabetes paitents and in the nondiabetes paitents,the FPG concentration were all significantly associated with LVEF(P0.05).Conclusion:The cardiac function status were significantly different among the CHF patients with or without diabetes in different admission FPG concentration groups.Compared to the CHF patients with diabetes,the elevated admission FPG concentration is common in patients without diabetes,and significantly correlated with cardiac function status.
