Abstract
Objective To investigate the diagnosis,surgery and prognosis of coronary artery myocardial bridge.Methods 9 males and 7 females of 12 to 71 years of age were enrolled for study.Systolic stenosis of coronary artery myocardial bridge was 60%~98% according to preoperative coronary angiography.There were 13 cases complicated with heart valve diseases,1 with congenital heart disease and 2 with isolated myocardial bridge.Myotomy on myocardial bridge was performed on 8 cases on beating heart,and 6 cases on arrested heart.Coronary artery bypass grafting was performed on 2 cases.Results Surgical treatment of coronary artery myocardial bridge were successful in all cases withou perioperative deaths.Significant improvement in myocardial ischemia was observed in ECG.Furthermore,significant increase of left ventricular ejection fraction(LVEF) was revealed after treatment [(52.5±4.3)% vs(65.9±5.5) %,t=6.9204,P0.05].Ventricular wall motion was improved in 3 cases.No coronary artery myocardial bridge was observed with coronary artery CT after 3 months in 6 cases.Conclusion Preoperative coronary angiography provides diagnosis of coronary artery myocardial bridge.The outcomes of surgical treatment is satisefactory for patients with more than 50% systolic stenosis of the coronary artery myocardial bridge.
