Abstract
Objective To investigate the pathogen and prognosis in children with acute diarrhea in Shanghai urban-rural integration region,and discuss the prevention issue.Methods Stool examination,bacterial culture and virus detection in children with acute diarrhea were performed in outpatient department from Dec.2005 to Nov.2006,whose course of disease 7 d,stool frequency3 times/d.Detailed history taking and physical examination,assessment of dehydration,appropriate treatment and follow-up were given.SPSS 13.0 software was used to analyze the data.Results A total of 314 cases were included (193 male and 121 female),and sex ratio was 1.6/1.0,aging from 42 days to 132 months.There were 77 cases (24.5%) of invaded (INV) diarrhea,106 cases (33.8%) of rotavirus(RV) diarrhea,19 cases(6.1%) of antibiotic-associated diarrhea(AAD),48 cases (15.3%) of food intolerance(FI) diarrhea and 64 cases (20.4%) of unknown causes.Diarrhea mitigation time:INV diarrhea(29.1±10.8) h, RV diarrhea(113.7±49.2) h,AAD diarrhea(83.0±25.8) h,FI diarrhea(67.0±20.1) h,unexplained diarrhea(76.0±20.3) h.INV diarrhea mitigation time and the recurrence rate of diarrhea in 6 months(11.7%) were lower than other times.RV diarrhea mitigation time and the recurrence rate of diarrhea in 6 months were highest(43.5%).Conclusions The incidence of INV diarrhea in the region decreased,while RV diarrhea gradually increased when compared with that of 1996-1998.Treatment and outcome of RV diarrhea is significantly different than other types.Children′s diarrhea is a kind of integrated disease caused by different causes,the pathogen is complicated.Attention to aetiological diagnosis is important for treatment in the rational use of antibiotics.
