Abstract
Summary chronic disease management through the process of changes from community full team model to family physician model to find in the follow-up of chronic disease management is facing the following problems:(I)follow-up of chrome heavy workload,relative lack of human resources;(2)community residents' awareness of the importance of chronic disease self- management is inadequate;(3)ignored high-risk chronic diseases management in chronic disease management;according to the above issues related to chronic diseases,to propose some countenneasures and discuss.
