Observation on curative effect of standard large trauma craniotomy in the treatment of severe craniocerebral injury of 48 cases

Abstract

【Objective】To observe the curative effect of standard large trauma craniotomy in the treatment of severe craniocerebral injury.【Methods】84 patients with severe craniocerebral injury,who were hospitalized in our hospital in recent 5 years,were randomly divided into 2 groups:standard large trauma craniotomy was performed in experiment group(n =48) and routine temporoparietal craniectomy was performed in control group(n =36).The con-ditions of postoperative recovery and follow up were observed and recorded.Subsequently,The incidence of complications,survival rate and mortality rate were comparatively analyzed.【Results】The total incidence of complications in experiment group and control group were 45.83%,69.44%,respectively.There was significant differences between them(χ2=4.65,P 0.05).Postoperative follow up of 6~12 months was performed.According to glasgow outcome scale(GOS),good and perfect rate and mortality rate were compared,with significant differences between(χ2=4.58,P 0.05;χ2=4.27,P 0.05).【Conclusion】Standard large trauma craniotomy can be applied in the treatment of severe craniocerebral injury with a variety of advantages such as sufficient exposure,thorough decompression,effective protection of cerebral function,and can significantly improved prognosis and reduced case fatality.
