Abstract
Objective To investigate the correlation between CYP3A5 genetic polymorphism and concentration / dosage( C / D) ratio of tacrolimus in 165 renal transplant patients who were tested 362times,and to provide the basis for individualized medication of tacrolimus through estimating the dosage according to different genotypes. Methods A total of 165 renal transplant patients who received tacrolimus,prednisone and mycophenolate mofetil for more than 6 months after renal transplantation were selected in this study. CYP3A5 genetic polymorphism was detected by PCR-RFLP. C / D ratio of tacrolimus in blood was compared among CYP3A5* 1 / * 1 genotype,CYP3A5* 1 / * 3 genotype and CYP3A5* 3 /* 3 genotype. Results The incidence of CYP3A5 * 3 allele was 77. 88% among renal transplant patients. The C / D ratio of tacrolimus in blood was( 40. 54 ± 19. 98),( 74. 76 ± 38. 71) and( 164. 33 ± 96.27) ng·mL- 1·mg- 1·d·kg in patients with CYP3A5* 1/* 1 genotype,CYP3A5* 1/* 3 genotype and CYP3A5* 3 / * 3 genotype,respectively. The C / D ratio of tacrolimus in patients with CYP3A5* 1 /* 1 genotype was significantly lower than that in patients with CYP3A5* 1 / * 3 genotype or CYP3A5* 3 / * 3 genotype( P 0. 001,P = 0. 021). No significant difference was found between CYP3A5* 1 / * 3 genotype and CYP3A5* 3 / * 3 genotype( P = 0. 132). Conclusion There is an obvious correlation between C / D ratio of tacrolimus and CYP3A5 genetic polymorphism. Compared with wild-type patients,the dosage of tacrolimus can be reduced in patients carrying CYP3A5* 3 allele.
