Mohnarin report of 2011: monitoring of bcterial resistance intensive care unit

Abstract

OBJECTIVE To determine the bacterial distribution and drug resistance of the pathogens isolated from ICU in China so as to guide the clinical use of antibiotics.METHODS Disc diffusion test,MIC test and E-test were used to detect the antimicrobial sensitivity of bacterial isolates from ICU patients.WHONET 5.6 was applied for analyzing the bacterial sensitive data from 49 tertiary Mohnarin member hospitals in China from Jan 1,2011 to Dec 31,2011.RESULTS A total of 15 752 bacterial strains were isolated in the survey period,which included 111 691(74.2%) strains of gram-negative bacteria,4061(25.8%)strains of gram-positive bacteria.Acinetobacter baumannii(3401 strains,21.6%),Pseudomonas aeruginosa(1996 strains,12.7%),Klebsiella pneumoniae(1752 strains,11.1%),Staphylococcus aureus(1630strains,10.3%) and Escherichia coli(1295 strains,8.2%) were of the five species of the most common isolates.The bacteria were mainly isolated from sputum,blood and urine.The methicillin-resistant S.aureus,S.epidermidis,and S.haemolyticus were 73.7%,86.1%,and 91.3%,respectively.No strains of vancomycin-resistant Staphylococci were detected.The resistance rates of Enterococcus faecium and Enterococcus faecalis to vancomycin were 5.1% and 0.5%.Enterobacteriaceae remained at a high sensitivity to carbapenems,The detection rates of the ESBLs-producing E.coli and K.pneumoniae were 79.6% and 70.1%,the non-fermenting bacteria were highly resistant to carbapenems antibiotics,the drug resistance rate of A.baumannii was over 75.0% to most of the test drug.CONCLUSION The non-fermenting bacteria,K.pneumoniae,S.aureus and E.coli are still the predominant pathogens in ICU in China.Bacterial resistance situation is still serious;more effective measures should be taken to control it.
