Abstract
[Objective] To evaluate the value of hepatectomy in radical operation for hilar cholangiocarcinoma.[Methods] The data of 75 patients with hilar cholangiocarcinoma who underwent resection was collected.According to the procedure of resection,75 cases were divided into two groups.In hepatectomy group,32 patients received hepatectomy combined with hepatoduodenal ligament skeletonization;In non-hepatectomy group,43 patients received local excision combined with hepatoduodenal ligament skeletonization.Subsequently,a comparative analysis was performed between the two groups in radical resection rate,survival rate,median survival time and complication incidence.[Results] Operation for hilar cholangiocarcinoma was successfully performed in the two groups without operative mortality.The statistically significant difference existed between the hepatectomy group and non-hepatectomy group with the radical resection rates of 78.13% and 25.58%,respectively(P 0.05).The statistically significant difference existed between the two groups with median survival times of 22.6 months and 13.5 months,postoperative 1-,2-,3-years survival rates of 81.48%,48.15%,29.63% and 54.29%,22.86%,8.57%,respectively(P 0.05).The complication incidences in the hepatectomy group and non-hepatectomy group were respectively 37.50% and 13.95% with statistically significant difference(P 0.05).[Conclusion] Hepatectomy combined with hepatoduodenal ligament skeletonization helps radical excision of hilar cholangiocarcinoma and increasing survival rate.
