Analysis of serum CK-MB detection results and its clinical significance in children

Abstract

Objective To study the false increase cause of serum creatine kinase MB isoenzyme( CK-MB) in children and its clinical significance. Methods The activity of serum CK and CK-MB was detected in patients by automatic biochemistry analyzer C501. The patients with CK-MB activity abnormal increase and CK-MB/ CK ratio over 25% were selected for further detection of the CK-MB mass concentration by immunochemiluminescence method,and the results were reviewed according to the clinical diagnosis. Results Total CK activity was low in 110 serum specimens of pediatric patients,while the false increase of CK-MB activity was found frequent in patients with liver disease,encephalopathy and hematologic tumor. CK-MB/ CK ratio below 5% was found in 24 cases,CK-MB/ CK ratio between 5% ~ 25% was found in 37 cases,and CK-MB/ CK ratio more than 25% was found in 49 cases. The CK-MB isoenzyme mass concentration was normal in 49 cases with CK-MB/ CK ratio more than 25%. Conclusion The limitations of immunoinhibition is likely to lead to the false increase of CK-MB activity in serum CK-MB determination of pediatric patients. When CK-MB/ CK ratio was over 25%,the false increase of CK-MB activity may be caused by the CK-BB,macro-CK in serum.
