Clinical application of the modified periotic incision in parotidectomy of benign tumor

Abstract

Objective To evaluate the clinical effect of the modified cosmetic periotic incision in parotidectomy of benign tumor.Methods Thirty-two patients with benign parotid tumor were reviewed in this study.Parotidectomy was designed and modified as follows:a modified periotic incision was used,and regional excision of parotid gland was applied;meanwhile the preservation of facial nerve as well as great auricular nerve was carried out in parotidectomy of benign tumor.The follow-up study included the cosmetic effect of this incision and the clinical effect.Results The modified cosmetic incision healed uneventfully in all patients without salivary fistulae.After a follow-up of 3-36 months,all the patients showed results with no obvious scar and sunken deformity left in face and submandibular region.One patient has presented with clinical Frey's syndrome.Five patients felt numb in ear lobe and the symptom was disappeared in 3-10 months post-operation.The facial nerve functional evaluation in 32 cases were gradeⅠ,and in 3 cases were grade Ⅱ.All the patients were highly satisfied with this modified incision.Conclusion The trauma and complications were reduced in the new technique.This modified periotic cosmetic incision could achieve a better facial cosmetic result in the parotid gland surgery.
