Abstract
[Objective] To study the clinical effect of the application of propofol target controlled infusion (TCI) in combination with bispectral index (BIS) monitoring feedback control in the elder patients undergoing painless colonoscopy. [Methods] Fifty patients with ASAI~II scheduled for electronic colonoscopy under propofol general anesthesia were enrolled in this study. All patients were randomly divided into two groups, including manual controlled infusion (MCI) group (n =25) and TCI -BIS group (n =25). In MCI group, all patients were received a single dose of propofol (7200 mg/h) intravenously till the patients'consciousness disappeared, whereafter received propofol (4mg/kg·h) intravenously. In TCI - BIS group, the patient's ages, weights and target concentration (CT) were loaded into the electronic infuser which could control the continuous infusion of propofol and adjust the infusion rate of propofol by BIS feedback. Vital signs including mean arterial pressure(MAP), blood oxygen saturation(SpO2), heart rate(HR), ECG and BIS value had been monitoring. Inducement time, recovery time, anesthesia effect and total propofol dose were observed and recorded. [Results] In TCI -BIS group as compared with MCI group, the inducement time was prolonged and total propofol dose was decreased significantly; during operation the MAP and HR of both the groups were decreased as compared with those before operation, but the most descending amplitudes of MAP and HR in TCI-BIS group was less than those in MCI group. [Conclusion] Propofol TCI anesthesia in combination with BIS monitoring feedback control in the elder patients undergoing colonoscopy can be achieved effectively, painlessly and safely ; the patients'hemodynamics and respiratory activity were more stable, so which must be worth extending and studying deeply.
