Abstract
Objective To investigate the factors influencing the prognosis of the patients with hepatocellular carcinoma(HCC) after curative hepatectomy.Methods The clinicopathological data of 246 patients with HCC after curative hepatectomy were retrospectively analyzed.The survival and recurrence rates were evaluated using the Kaplan-Meier method and multivariate analysis was performed by Cox proportional hazard model to determine the independent prognostic predictor.Results The median survival time of the 246 patients was 18.3 months with the overall 3-and 5-year survival rates of 43.0% and 39.5%,and the overall 3-and 5-year recurrence rates of 69.4% and 71.6%,respectively.The multivariate analysis revealed that tumor diameter,tumor capsule,portal vein tumor thrombus and intraoperative blood transfusion were the independent factors for influencing the survival of HCC.Tumor diameter,tumor capsule,satellite focus,portal vein tumor thrombus and intraoperative blood transfusion were the independent factors for influencing the recurrence of HCC.Conclusion Tumor diameter,tumor capsule,satellite focus,portal vein tumor thrombus and intraoperative blood transfusion are the independent factors related to the prognosis in HCC patients after curative hepatectomy.Early detection and standardized surgery are important to improve the outcomes of the patients with HCC.
