Abstract
Objective To compare the resuscitation effect of lactated Ringer's solution(LR) combined with hydroxyethyl starch(HES),whole blood or red blood cells on hemorrhagic shock rats.Methods SD rats(n=24) were inflicted with a sharp resection of 75% of the tail to initiate the hemorrhage.When the blood pressure(BP) was decreased to 40 mmHg,LR solution was infused to the rats for 1 h.Then the animals were randomly divided into 3 groups,LR+HES(group Ⅰ),LR+HES+auto-whole blood(group Ⅱ),and LR+HES+auto-red blood cells groups(group Ⅲ),and received the corresponding infusion respectively for 2 h when the BP was increased to 80 mmHg.The blood samples were collected before shock,1 h after shock and 2 h after resuscitation.Hematologic and hemodynamics parameters,such as blood cell count,heamoglobin,hematocrit,platelet count,heart rate,BP,survival time,left intraventricular systolic pressure(LVSP),maximal change rate of left intraventricular pressure(±dp/dtmax),were measured and recorded.Results Hematologic parameters of each group had significant difference in 1 h after shock compared with 2 h after resuscitation,and the 2 time points compared with before shock.Those of group Ⅱ and group Ⅲ were significantly higher than those of group Ⅰ.In every group,the parameters at the time of 2 h after resuscitation had significant difference to those at 1 h after shock.Group Ⅱ and group Ⅲ had a significantly longer survival time compared with group Ⅰ.HR,LVSP and ±dp/dtmax in each group at 1 h after shock and 2 h after resuscitation were significantly decreased compared with before shock.Those of group Ⅱ and group Ⅲ were significantly higher compared with group Ⅰ;those of each group in 2 h after resuscitation were markedly increased compared with 1 h after shock.Conclusion LR solution plus HES has efficient effect on the resuscitation of hemorrhagic shock rats to maintain normovolemia,and the infusion of auto-red blood cells is helpful to correct anemia.Plasma-poor red cell concentrates is safe and causes no abnormal phenomena of blood clotting.
