Application Value of Serum C-reaction Protein and Prealbumin Level in the Prognosis Prediction of Elderly Patients with Chronic Obstructive Pulmonary Disease

Abstract

Objective: To study application value of C-reaction protein and prealbumin in elderly patients with chronic obstructive pulmonarydisease.Methods: 121 patients with COPD were collected in the emergency department of our hospital.The CRP and PA value were checked in the first day and the seventh day,and according to the prognosis the patients were divided into the death group and the sur-vival group.The difference of CRP and PA levels were compared with the two groups in the first day and the seventh day;According to the severity of COPD patients were divided into 4 groups,using Spearman rank correlation analysis method to study serum CRP,PA levels and the severity of COPD correlation.Meanwhile,CRP,PA levels and the prognosis of patients were studied using Logistic regression analysis.Results: The serum CRP level of the survival group in the seventh day was significantly lower than that in the first day,but serum PA level increased(P0.05),and there was significant difference between the death group and the survival group(P0.05).No significant difference of the serum CRP and PA level in the first and seventh day was found between the death group and the survival group(P0.05);The serum PA level of death groups in the seventh day decreased significantly as compared with that in the first days(P0.05),serum CRP level in the first day was not significantly different from that in the seventh day(P0.05);Spearman rank correlation analysis showed that serum CRP level was positively correlated with severity of COPD(r=0.357,P0.05),serum PA level was negatively associated(r=-0.392,P0.05),Logistic regression analysis showed that the rise of serum CRP level was an independent risk factor of the prognosis of COPD patients(OR=1.32,95%CI: 1.02~2.38),while the rise of serum PA level was an protective factor of the prognosis of COPD patients(OR=0.94,95% CI: 0.05~1.06).Conclusion: The serum CRP and PA levels were correlated with the severity in elderly patients with COPD.Dynamic changes of CRP and PA can predict the severity and prognosis of elderly patients with COPD.
