Abstract
Objective:To compare the efficacy of GnRH agonist short protocol and GnRH-antagonist protocol on the outcome in poor-responder patients undergoing controlled ovarian hyperstimulation for in vitro Fertilization and embryo transfer.Methods:Seventy-two poor-responder patients were randormized to treat by either the a flexible GnRH antagonist protocol(n=29)or GnRH agonist short protocol(n=43)for controlled ovarian hyperstimulation, the duration and dose of gonadtrophine as well as the number of oocytes retrieved, embryo quality, fertilization, implantation, and pregnancy rates were evaluated. Results:There was no cycle cancellation in the GnRH antagonist protocol and 13.9% cancellation rate in the GnRHa short protocol due to lack of response or premature LH surge.A significantly higher LH and E2 lever on the day of HCG administation in the short protocol. No significant difference was found in the duration and dose of gonadtrophine as well as the number of oocytes retrieved, embryo quality, fertilization, implantation, and pregnancy rates between the two groups. Conclusions:As for the outcome of IVF-ET for the patients with poor responder undergoing controlled ovarian hyperstimulation, the GnRH antagonist protocol was not better than the GnRHa short protocol,But in order to reduce the cancell rate, the GnRH antagonist protocol maybe a good considertation.
