Abstract
Objective To investigate the therapeutic effect of sulodexide combined with losartan on patients with early diabetic nephropathy. Methods Sixty-five patients with early diabetic nephropathy were randomly divided into three groups:the sulodexid group (600 LSU qd,r im for 4 weeks,250 LSU bid,po for 4 weeks); losartan group (50 mg qd,po for 8 weeks); combine therapy groups (sulodexid 600 LSU qd,r im for 4 weeks,250 LSU bid,po for 4 weeks and losartan 50 mg qd,po for 8 weeks). Three groups were treated with the same basic diet control and routine treatment,and the levels of urinary albumin excretion rate (UAER),urea nitrogen creatinine and plasminogen activator inhibitor-1 were determined. Results After eight-week treatment,the level of UAER was decreased significantly in three groups (P0.05),and was reduced by 28.53%,26.67% and 43.72% in sulodexide group,losartan group and the combine therapy group,respectively. Significant difference was noted in the level of UAER between sulodexide / losartan groups and combine therapy group (P0.05),but no significant difference was observed between losartan group and sulodexide group (P0.05). After treatment,the level of PAI-1 in the three groups was decreased to some extent; there were statistically significant differences in the level of PAI-1 in the three groups between before and after treatment (P0.05),while no statistically significant difference was found in the levels of SCr and BUN of three groups between before and after treatment (P0.05). Conclusions Sulodexide combined with cozaar could decrease the level of UAER in patients with early diabetic nephropathy significantly,and is more effective than the application of Sulodexide or losartan solo.
