Abstract
Objective To study the different effect of systolic and diastolic pressure in middle aged and elderly with hypertension in rural. Methods A cluster sampling cross sectional survey was carried out on rural residents over 35 years old in three gorge dam area. Results In uncontrolled hypertension, the percent of systolic and diastolic hypertension was 46.3% and the percent of isolated systolic hypertension was 37.9% and the percent of isolated diastolic was 15.8%. Hypertension awareness and treatment in older group was higher than that in younger group. The accurate percent of staging by systolic blood pressure alone was higher than that of staging by diastolic blood pressure alone in two age groups. Subjects in older group who had isolated systolic hypertension and systolic and diastolic hypertension required a greater reduction of systolic pressure than that in the younger group to attain a systolic blood pressure treatment goal. On the other hand, the diastolic blood pressure reduction in younger group of isolated diastolic hypertension was higher than that in the older group. Conclusions Systolic and diastolic hypertension and isolated systolic hypertension were the majority subtype of uncontrolled hypertension in three gorge dam rural area. The elevation of systolic pressure was the main problem in uncontrolled hypertension.
