Noninvasive positive pressure ventilation in the treatment of severe respiratory failure complicated with multiple organ dysfunction syndrome at moderate plateau

Abstract

Objective:To evaluate the efficacy of noninvasive positive pressure ventilation(NPPV) for patients with severe chronic obstructive pulmonary disease(COPD) complicated with multiple organ dysfunction syndrome(MODS)at moderate plateau.Methods: A retrospectively study was made in eighteen patients with severe respiratory failure resulted from COPD or/and severe trauma/infection treated with NPPV(NPPV group,n=18);In addition,a group of 14 COPD patients treated with conventional method was assigned as control group(non-NPPV,n=14).Results: Before treatment,the clinical risk factors and blood gas parameters in NPPV group were more serious than that in non-NPPV group(P0.05).However,the treatment outcome in NPPV group was significantly better than that in non-NPPV group(P0.05).Conclusion: The early-stage NPPV therapy at moderate plateau can improve clinical symptoms and signs,and decrease hospital mortality rate both in type Ⅰ and Ⅱ respiratory failure patients.NPPV might be an effective treatment method for preventing MODS in patients with severe respiratory failure.
