Abstract
Objective To study the risk factor which influencing postoperative bilirubin level in the patients with hepatocellular carcinoma during perioperation. Methods 294 patients who have been performed surgical therapy of HCC were enroll except obstructive jaundic preoperation,,to find the risk factor which affected postoperative bilirubin level.Compare the difference between the patients whose postoperative bilirubin level≥51.3 mmol/L and patients whose postoperative bilirubin level51.3 mmol/L in the risk factor and short-term outcome.Result Postoperative bilirubin level is apparently correlated with preoperative bilirubin level, liver cirrhotic degree, operation time, blood loss volume.48 patient's bilirubin level are51.3 mmol/L after operation, which is with higher level in preoperative bilirubin(P0.001),more severe liver cirrhotic (P0.001),longer operation time(P0.001),more blood loss volume(P0.01),longer inhospital days(P0.01),more complication and mortality(P0.01).Conclusion ①Perioperation bilirubin level have a significant short-term outcome value in the patients of HCC.②Performing operation should select patients with slight hepatic cirrhosis or non-cirrhotic liver and well preserved hepatic function, control operation time, reduce blood loss volume, properly treat the postoperative obstructive jaundice, intensify perioperative care.
