Investigation of Clinical Classification of the Secondary Deformities after Prim ary Repair of the Bilateral Cleft Lip/Bilateral Cleft Lip and Cleft Palate.

Objective:To investigate deforming characteristics and cl assification of the secondary deformities after primary repair of the bilateral cleft lip and bilateral cleft lip and cleft palate.Methods:Fifty patients with secondary deformities after primary repair of the bilatera l cleft lip and bilateral cleft lip and cleft palate from Department of Oral and Maxillofacial Surgery of Xiangya Hospital,Central South University were observe d their 19 secondary deforming contents on the nose,lip and occlusion and jaw.Th ese contents include flattened nasal tip, displacement of the alar cartilages,as ymmetrical nastrils,no nostril sill, short columella.tight upper lip, loose uppe r lip, no Cupid's bow and Cupid's bow peak, no median tubercle, whistling lip de formity.reversed overbite in the anterior teeth, displaced or missed canines, cl eft alveolar bridge, small dental arch, disorder of the dentition, no anterior a lveolar bridge and retruded anterior maxillae.Results:Fift y patients with the secondary deformities after primary repair of the bilateral cleft lip and bilateral cleft lip and cleft palate all have the deformities of t he flattened nasal tip, displacement of the alar cartilages, asymmetrical nostri ls, no nostril sill, short columella, no Cupid's bow, no Cupid's bow peak and th e whisting lip deformity.Forty-two patients of them have the cleft alveolar brid ge and the disorders of the dentition.Thirty-three patients repaired the bilater al cleft lip and the bilateral cleft lip and cleft palate to use Barcky's method all have reversed overbite in the anterior teeth, the disorder of the dentition and the retruded anterior maxillae.Twenty-eight patents of 50 patients have sho wed the whisting lip deformity.Conclusions:The patients af ter primary repair the bilateral cleft lip and cleft lip and cleft palate presen t the nasal、lip、occlusial and maxillary deformities, which must be managed by using the secondary repair.
