Hepatoduodenal ligament lymph nodes dessection in the radical gastrectomy for gastric cancer

Abstract

Objective: To determine metastases rates of lymph nodes in the hepatoduodenal ligament in gas-trectomy for gastric cancer. Methods: Forty six gastric cancer cases were operated during Oct 2002 to Sept 2004 with total gastrectomy and D2+ lymphadenectomy, including hepatoduodenal ligament lymph nodes dessection (the 12th group lymph nodes, classification by Japanese procedure). Among them 6 were stage Ⅰ, 6 stage Ⅱ, 26 stage Ⅲ and 8 were stage Ⅳ according to TNM staging. Results: The results showed that the metastases rate in lymph nodes around the hepatic artery (12a) was 26. 1%, in lymph nodes around the common bile duct (12b) was 13. 1% and behind the port vein (12p) was 2. 2%. The metastases rate in lymph nodes around the hepatic artery (12a) was more high than the others. The metastases rate in the upper of stomach, in the central part of Stomach, in the lower part of Stomach and the whole stomach cancer was 16. 7%, 28. 6%, 14. 8% and 83. 3%, there is significant different metastases rate between the whole stomach cancer and the part stomach cancer (P 0 05); The metastases rate of lymph nodes in the hepatoduodenal ligament was different among the different tumor size , when the tumor of the size was ≤ 5 cm or 5 cm, the metastases rate was 13. 8% and 47. 1%, there is significant difference (P 0 05); when the tumor was penetrating to serosa or was not penetrating to serosa, the metastases rate of lymph nodes in the hepatoduodenal ligament was 35. 5% and 0. 0%, there is significant difference (P 0 05); when the tumor was bad differentiation, the metastases rate of lymph nodes in the hepatoduodenal liga- ment was 29. 7% , and when the tumor was good differentiation, the metastases rate of lymph nodes in the hepatoduodenal ligament was 11. 1%, there is significant difference (P0 05). Conclusions: The metastases rate of lymph nodes in the hepatoduodenal ligament is relevant with location, size, differentiation and penetrating to serosa of the tumor.
