Anatomical study and application of supraorbital keyhole approach

Abstract

Objective To study the microanatomy of supraorbital keyhole approach and its application in the treatment of intracranial aneurysms of anterior circulation and tumors in sellar region.Methods Supraorbital keyhole approach was simulated in 10 adult cadaveric heads under microscope.A total of 76 cases including 52 of intracranial aneurysms of anterior circulation and 24 of tumors in sellar region were performed surgery by the approach.Results Via supraorbital keyhole approach,the anatomical structures of sellar region and Willis circle were observed precisely.All 52 cases of aneurysms were successfully clipped,including 49 of good recovery and 3 of moderate disabled,and no dead case.Among 24 tumors in sellar region,22 were totally resected and 2 were subtotally resected.No complication occurred.Conclusion Supraorbital keyhole approach is widely used in clinical practices.It is safe,effective and minimal invasive for the treatment of aneurysms of anterior circulations and tumors in sellar regions.
