Clinical study for posterolateral approach combined with posteomedial approach for treatment of tibial plateau posterior column fracture

Abstract

Objective To observe the surgical methods and clinical efficacy of the posterolateral approach combined with posteomedial approach for treatment of tibial plateau posterior column fracture.Methods Thirteen patients with tibial plateau posterior column fracture were treatmented with posterolateral approach combined with posteomedial approach open reduction and internal fixation.The tibia platform varus angle(TPA)and posterior angle(PA)were measured immediately and 12 months after operation.To evaluate the clinical effect with HSS knee score and range of motion(ROM)after operation.Results All of 13 patients were followed up for 12~20 months and with fractures healed.The HSS knee score was 73~92 points,an average of(82.4±6.8) points,and ROM was(121.6±1.2)°.TPA and PA degree had no significant difference(P 0.05) in immediately and 12 months after operation.Conclusion The posterolateral approach combined with posteomedial approach for the treatment of tibial plateau posterior column fracture is mainly suitable for extensive and complex fractures,and dual incisions facilitates the posteromedial and posterolateral fracture reduction and fixation and recovery of joint function,with little soft tissue trauma.
