Clinical research of treatment for T_ (3~4)N_(0~1)nasopharyngeal carcinoma with radiotherapy combined with chemotherapy.

Abstract

Objective To investigate the clinical effect of radiotherapy combined with chemotherapy on T_ 3~4 N_ 0~1 nasopharyngeal carcinoma patients.Methods Patients with T_ 3~4 N_ 0~1 were divided into radiotherapy combined with chemotherapy group ( study group)and radiotherapy group ( control group). Patients in study group were treated with radiotherapy combined with chemotherapy included cisplatin 30~40mg and 5-Fu 500~750mg, Once a week. All patients received late course hyperfractionated radiotherapy to a total doses of 74~80Gy in nasopharynx.Results Six months after radiotherapy, complete response(CR) rates of nasopharynx were 84.61% in study group and 62.16% in control group(P=0.0419). The 1, 2, 3-year local control rate were 100.00%, 92.30%, 74.35% and 100.00%, 78.37%, 51.35%, respectively. The 1, 2, 3-year survival rates were 92.30%, 84.61%, 76.92% and 86.48%, 67.56%, 54.05%, respectively. There were no significant difference between the two groups in the 1, 2-year local control and survival rates. The 3-year local control and survival rate of study group were higher than those of control group (P0.05).Conclusion Local control was dominant for patients with T_ 3~4 N_ 0~1 , combination of low dose chemotherapy with late course hyperfractionated radiotherapy had better therapeutic effect.
