The study on the efficiency of phacoemulsification with posterior chamber intraocular lens in the management of chronic angle-closure glaucoma with cataract

Abstract

Objective:To discuss the efficiency of phacoemulsification with implanting posterior chamber intraocular lens(PC-IOL)in the management of chronic angle-closure glaucoma with cataract. Phacoemulsification surgery plus PMMA PC-IOL implantation with scleral tunnel incision performed on 38 eyes(26 cases) with chronic angle-closure glaucoma and cataract was retrospectively studied.All patients (57 to 76 years old,mean 63.5±6.1) had had anti-glaucomatous surgeries,the postoperative time were from 6 to 27 years,mean (11.3±5.8)years.The postoperative follow-up was from 3 to 12 months,mean (6.2±2.7) months. Results:The mean postoperative IOP (14.38±4.73 mmHg) was significantly decreased compared to preoperative IOP (17.62±5.39mmHg,t=2.79,P0.01).The central anterior chamber depth improved from preoperative 1.72±0.36 mm to postoperative 2.87±0.29 mm (t=15.34,P0.001).The peripheral anterior chamber depth (PACD) was≤1/4 CT (corneal thickness) in all 38 eyes,the postoperative iris bulge,however, disappeared in all eyes with PACD≥1CT (P0.001,Ridit analysis).The postoperative best corrected visual acuities were improved significantly in 35 eyes (92.1%) (P0.001). Conclusions:Phacoemulsification with PC-IOL implantation can be a good alternative in treating chronic angle-closure glaucoma with cataract.
