Abstract

Objective To investigate the bacterial distribution and antibiotic resistance of wound infections in 114 hospitals during 2009 in China.Methods Disc diffusion test,MIC test and E-test were used to study the antimicrobial resistance.WHONET 5.5 was applied for analysis the antibacterial sensitive data.Results A total of 6171 isolates were isolated from wound infections,including 2728(44.2%) Gram-positive strains and 3443(55.8%) Gram-negative strains.The top six pathogens were Staphylococcus aureus(19.8%),Escherichia coli(16.9%),Pseudomonas aeruginosa(8.9%),Klebsiella pneumoniae(8.0%),Staphylococcus epidermidis(6.7%) and Acinetobacter baumannii(5.2%).Methicillin-resistant strains in Staphylococcus aureus and Staphylococcus epidermidis accounted for 31.0% and 65.1%,respectively.No Staphylococcus spp.was intermediate or resistant to vancomycin and linezolid.The intermediate rate of Staphylococcus epidermidis to teicoplanim was 1.2%.Enterococcus faecium had higher resistant rates to almost all the antibiotics than Enterococcus faecalis.The resistant rates of Enterococcus faecalis and Enterococcus faecium to vancomycin(VRE) were 0.5% and 1.5%.One point three percent of Enterococcus faecium were resistant to teicoplanin.No Enterococcus isolate was resistant to linezolid.ESBLs-producing strains accounted for 65.5% in Escherichia coli and 40% in Klebsiella pneumoniae.Escherichia coli had higher resistant rates to almost all the antibiotics than Klebsiella pneumoniae.More than 43% of Acinetobacter baumannii was resistant to most of the tested drugs.The resistance rate of Pseudomonas aeruginosa and Acinetobacter baumannii to imipenem was 17.2% and 33.8%,respectively.Conclusion Escherichia coli,Staphylococcus spp.and Pseudomonas aeruginosa were predominant pathogens in wound infections in China.The bacterial resistance was a serious problem in China.The resistance of pathogens should be monitored to provide practical guidance of rational selection of antibiotics for clinicians.
