Abstract
OBJECTIVE The aim of the present study was to investigate polysomnography(PSG)determinants of excessive daytime sleepiness(EDS)in patients with obstructive sleep apnea-hypopneasyndrome(OSAHS), and to find related factors which will predict OSAHS patients having daytime sleepiness and investigate the mechanism of daytime sleepiness. METHODS A total of 319 suspected OSAHS cases were enrolled in this study. There were 39 cases of simple snoring, 71 cases of mild OSAHS, 63 cases of moderate OSAHS, 146 cases of severe OSAHS, 34 patients of non-nocturnal hypoxemia, 55 patients with mild nocturnal hypoxemia, 163 patients with moderate nocturnal hypoxemia, and 67 patients with severe nocturnal hypoxemia. The Epworth sleepiness scale (ESS)scores and polysomnography monitoring data were analyzed. The average ESS scores of the patients were compared among groups. The correlation of ESS scores with percentage of time when oxygen saturation lower than 90%(SLT90%), microarousal index, number of oxygen desaturation 4% per hour(ODI4)was analyzed. According to the ESS scores, the patients were classified into 84 patients with EDS and 235 without EDS. Multiple logistic regression analysis were performed on age, BMI, ODI4, AHI, Lowest oxygen saturation(LSaO2), SLT90%. RESULTS The mean ESS scores were significant statistically difference among groups. The ESS scores had a negative correlation with the LSaO2 and a positive correlation with SLT90%, microarousal index and ODI4. Multiple logistic regression analysis revealed that EDS was related to AHI, BMI and LSaO2. CONCLUSION AHI, BMI and LSaO2 are related factors of excessive daytime sleepiness in patients with OSAHS, which will predict OSAHS patients having daytime sleepiness.
