Meta-analysis of Fasudil Hydrochloride vs.Nimodipine in the Treatment of Cerebral Vasospasm after Subarachnoid Hemorrhage

Abstract

OBJECTIVE: To evaluate the short-term efficacy and safety of fasudil hydrochloride vs.nimodipine in the treatment of cerebral vasospasm after subarachnoid hemorrhage.METHODS: They searched relevant randomized controlled trials(RCT) from Cochrane library,PubMed,EMBASE,VIP,CBM and CNKI.The search covered from the establishment of database to Feb,2011.Other retrieval methods were adopted.Randomized controlled trials of fasudil hydrochloride vs.nimodipine on cerebral vasospasm after subarachnoid hemorrhage were included;Trial data were reviewed and extracted independently by 2 reviewers.They evaluated the quality of the included studies using RevMan 5.0 software.RESULTS: 14 RCT were included.Meta analysis results showed that: compared with nimodipine,fasudil hydrochloride had a significant benefit in improving short-term curative effect [OR=2.69,95%CI(1.76,4.13),P0.000 01] and could also improve the speed of cerebral arteries [MD=12.11,95%CI(6.86,17.36),P0.000 01].However,compared with nimodipine,it had not a significant benefit in reducing vasospasm.Present evidences showed fasudil hydrochloride could not decrease the occurrence of fever,palpitation,facial flushing and headache,etc.CONCLUSION: Compared with nimodipine,fasudil hydrochloride can significantly improve curative effect.Meanwhile,the bias from above conclusions maybe exists,and it is still necessary to design larger,scientific clinical studies to prove the truth.And it is worthy of clinical application and the reference to the research.
