Clinical analysis of 82 chronic suppurative otitis media in children

Abstract

Objective To explore the different and common features of different types of in-patient chronic suppurative otitis media(CSOM) in children in clinical characteristics,complications and operations.Methods Clinical cla-ssification was performed in 82 in-patient children cases(102 ears)with CSOM according to otologic test,CT scanning,conditions in operation and pathology.Clinical types were further certified following preoperative clinical data,extent of disease,range of auricular bone lesion and operation pattern.Results Of all the 102 ears,97 accepted surgery.In 5 cases,only one ear underwent surgery with both sides involved.Fifty ears were found to be CSOM,and 47 to be cholesteatoma type of CSOM according to condition of chain of ossicles and pathology.Tympanic membrane was invisible in 37 ears(36.3%) as a result of granulation and cholesteatoma in external auditory canal.As for the type of operation,radical mastoidectomy was performed in 29 ears(29.9%),modified mastoidectomy in 27 ears(27.8%),tympanoplasty in 20 ears(20.6%),mastoidectomy and tympanoplasty in 19 ears(19.6%),modified mastoidectomy and facial nerve decompression in 2 ears(2.1%).Conclusions Cholesteatoma type was common in children CSOM.Because of extensive lesion,the primary goal of operation was to eliminate disorders thoroughly,and then to re-establish hearing.
