Abstract
Objective To study the application value and prognosis evaluation of SVV monitoring in liquid recovery of patients with septic shock. Methods Total of 60 patients with septic shock treated in Department of ICU of People's Hospital of Wuhan University from Feb. 2009 to Feb. 2013 were included as the research objects. All patients were divided into high SVV group and low SVV group according to the value of SVV. The correlation of SVV,central venous pressure( CVP) and after liquid load cardiac index value increase( Δ CI) of patients with septic shock liquid recovery before and 6 h after treatment were evaluated,and the 28 d survival rate,ICU hospital stay length,time of mechanical ventilation,time of vascular active drug discontinuation of two groups were compared. Results There were significant difference of MAP,CVP,CI,GEDVI,ITBVI,SVV,lactic acid,clearance,and BE before and after fluid resuscitation( P 0. 05). SVV had a positive correlation with Δ CI( r = 0. 744,P 0. 05),but CVP had no correlation with Δ CI( P 0.05). Compared with low SVV group,the 28 d survival rate of high SVV group was obviously higher,the ICU hospital stay length,mechanical ventilation time and vascular active drug discontinuation time were significantly shortened( P 0. 05). Conclusion SVV can reflect liquid reactivity during liquid recovery in patients with septic shock,and can be used as a index for guiding liquid treatment and prognosis.
