Abstract
Objective To investigate the effects of remifentanil preconditioning on serum S-100β protein during pump-assisted beating heart coronary artery bypass grafting(CABG).Methods Fourty patients were randomized into two groups of R(receiving remifentanil preconditioning)and C(without remifentanil preconditioning).Group R was divided to three subgroups of R1(remifentanil 0.6 g·kg-1·min-1),R2(remifentanil 1.2 μg·kg-1·min-1)and R3(remifentanil 1.8 μg·kg-1·min-1)with 10 cases each.Remifentanil infusion started at 30 min after anesthesia induction in the manner of 5 min infusion and 5 min interval for three times.Group C was given normal saline instead of remifentanil as the control.S-100β protein in jugular bulb blood was detected before induction(T0),before CPB(T1),at 30 min after the beginning of CPB(T2),and at the end of CPB(T3).The neuropsychological status was evaluated with mini-mental state examination(MMSE)before and at 24 h after operation.Results The difference of MMSE score before and 24 h after operation was less in group R3 than that in the other three groups(P0.05).S-100β levels of 4 groups were higher at T2 and T3 than those at T0 and T1(P0.01).S-100β level at T2 and T3 was lower in group R3 than that in the other three groups(P0.05).Conclusion Remifentanil 1.8 μg·kg-1·min-1 preconditioning has a protective effect against brain injury.
