Diagnostic value of susceptibility weighted imaging in acute cerebral infarction with cerebral microbleeds and research on related clinical factors

Abstract

Objective: To study the diagnostic value of susceptibility weighted imaging(SWI)in acute cerebral infarction with cerebral microbleeds(CMBs)and risk factors of acute cerebral infarction with CMBs.Methods:A total of 62 cases of acute cerebral infarction underwent SWI,conventional MRI,and DWI at 3.0T MR scanner(GE Signa HDX).Combined the related clinical data,The following analyses were performed.①To record the detection rate and distribution of SWI data in acute cerebral infarction with CMBs cases.②To evaluate the association of acute cerebral infarction with CMBs and related clinical variables,including gender,age,hypertension,diabetes mellitus,hyperlipidemia,heart diseases by univariate analysis and multivariate logistic regression,and determine the risk factors in acute cerebral infarction with CMBs.Results:The detection rates of acute cerebral infarction with CMBs were 0.00%(conventional MRI),8.06%(DWI),40.32%(SWI),respectively.There was significant difference among these different MR scanning sequences.The detection rate of CMBs by SWI was higher than that by other sequences.2.Based on SWI images,all 25 cases of CMBs were multiple.The most common locations of CMBs in patients with acute cerebral infarction were basal ganglia and thalami,followed by cortico-subcortical region,brain stem and cerebellum.3.Age and hypertension were associated with the presence of CMBs in acute cerebral infarction(P0.05)and age(OR=1.704,95%CI 0.029-2.840;P0.001)and hypertension(OR=1.039,95% CI 1.003-1.588;P=0.019)were independent risk factors for the presence of CMBs,whereas gender,diabetes mellitus,hyperlipidemia and heart disease were not associated with the presence of CMBs in acute cerebral infarction(P0.05).Conclusions:① SWI is superior to conventional MRI and DWI in diagnosis of acute cerebral infarction with CMBs,and it is the most sensitive and valuable examination method in the detection of CMBs.②Age and hypertension are independent risk factors for the presence of CMBs.
