Impact of family hypertension history on blood pressure variability in hypertensive patients

Abstract
Objective To investigate blood pressure variability and the influence on target organs of directly-related members with hypertension family history.Methods 102 patients were divided into two groups,with hypertension family history(hypertension family history,HF +,n=45) group;with no hypertension family history(HF-,n=57) group.Physical examination,biochemical tests,echocardiography and ambulatory blood pressure were completed.Results In a variety of levels of cardiovascular risk factors,the difference was not statistically significant between the two groups of patients.The average daytime diastolic blood pressure,daytime diastolic blood pressure standard deviation,daytime mean arterial pressure standard deviation and average diastolic blood pressure at night,24 h average diastolic pressure,24 h diastolic blood pressure standard deviation,and 24 h mean arterial blood pressure standard deviation of HF+ group were significantly higher than those of HF-group(P0.05).In average daytime systolic blood pressure,average daytime systolic blood pressure standard deviation,nighttime average systolic blood pressure,nighttime systolic blood pressure standard deviation,the difference was not statistically significant between the two groups(P0.05).The incidence of nondipping blood pressure rhythm in HF+group was significantly higher than that in HF-group(51.11% vs.26.31%,P=0.010),the occurrence rate of left ventricular hypertrophy in HF+group was significantly higher than that in HF-group(68.89 % vs.42.10%,P=0.007).After all inspection,patients were gave oral antihypertensive drugs to control the blood pressure standardly.The proportion of using 3 or more than 3 of the first-line antihypertensive drugs in HF+group was significantly higher than that in HF-group(53.33% vs.29.82%,P= 0.016).Conclusions The patients with hypertension family history begin with an early age of onset and blood pressure variability were increased.Target organ damage may be more susceptible in these patients.
