Feasibility Study of Continue Nursing Service After Discharge from Hospital for Geriatric Patients

Objective Through 150 cases of elderly patients for continuing nursing care after discharge,to study the effect on prognosis and feasibility analysis of continuing nursing care after discharge.Methods 300 cases of elderly patients in our hospital from 2010 October to 2012 October were randomly divided into the observation group and the control group,150 cases in each group.Two groups of patients were given clinical symptomatic treatment and routine nursing measures,based on these,the observation group were given continuing nursing care after discharge from hospital.For patients after discharge from hospital,for a one-year follow-up investigation,compare compliance,quality of life,mental state,complications of the two groups patients.Results Including the return to the hospital to check,reasonable diet,exercise therapy,rational medication method,self monitoring of blood pressure,blood glucose,several aspects of the behavior of medical compliance of the observation group were significantly higher than those of control group(all P0.05).After intervention,quality of life and psychological state of observation group was better than those of the control group(P0.05),Complications in the observation group was significantly lower than those in the control group(P0.05).Conclusion Lasting nursing after discharge is an effective nursing measures,and effectively promote the improvement and rehabilitation of patients,reduce the incidence of complications risk,which is worth in the clinical work further promotion.
