Protective effect of propofol and fluctose-1,6-diphosphate on the liver of piglet undergoing auxiliary liver transplantation

Abstract

Objective:To observe the protective effect of propofol and fluctose-1,6-diphosphate(FDP) on the liver of piglets undergoing auxiliary liver transplantation.Methods:Twenty-four healthy piglets,weighing 20-35 kg,were divided into 3 groups according to pair of littermates,with the lighter one as the donor and the heavier as the recipient.Control group(Group C,n=4),propofol group(Group Y,n=4)and experimental group(FDP + propofol,Group R,n=4).After intramuscular injection of ketamine 10-15 mg/kg,anaesthesia was induced with ketamine(1 mg/kg),fentanyl(4 μg/kg) and vecuronium(0.2 mg/kg).The piglets were mechanically ventilated after tracheal intubation,with maintenance of PETCO2 at 35-45 mmHg.Anesthesia was maintained with 2% enflurane and intermittent infusion of vecuronium 0.1 mg/kg.In group Y,R and C,upon the donor liver exposure and right before the organ was planted,0.9% sodium chloride,10% FDP 2 ml/kg and 0.9% sodium chloride 2 ml/kg were separately administered within 1 h.Propofol at dose of 5-8 mg/(kg·h) was infused continuously during operation in both group Y and R,while 0.9% sodium chloride was used in group C instead.MAP,HR,SpO2 and EKG were monitored continuously.To determine the MDA,SOD,GSH-Px,T-AOC,ALT and AST concentration,right internal jugular vein blood samples of 3 ml were taken at the start of operation(T1),right after the liver was removed(T2),at 30 min after the liver removal(T3),at 5 min(T4) and 60 min(T5) after reperfusion,and at the end of operation(T6).For ALT and AST concentration determination,right internal jugular vein blood samples of 2 ml were taken at 2 h(T7) and 24 h(T8) after the operation being over.Results:As compared with T1 in 3 groups,the level of SOD at T3 in group C and the level of SOD at T3-6,MDA at T4 in group R and Y group were higher(P0.05).ALT at T5-8 in the 3 groups and AST at T4-8 in group C and at T5-8 in group R and Y were totally increased(P0.05).SOD levels at T4-6 were boosted,whereas MDA at T5,6 were decreased in group R and Y compared with group C simultaneously.ALT level at T6-8,AST at T5-8 in group R and Y were lower than those at the same time point in group C(P0.05).The level of SOD at T6 was higher,and yet lower MDA at T6,ALT and AST at T7,T8 were seen in group R than in group Y(P0.05).Conclusion:Propofol and FDP can,to some extent,protect the livers of piglets during auxiliary liver transplantation.
