Comparison between cataract phacoemulsification combined with goniosynechialysis and trabeculectomy for primary angle-closure glaucoma

Abstract
Objective To explore the clinical effects of cataract phacoemulsification combined with goniosynechialysis and trabeculectomy for primary angle-closure glaucoma ( PACG) .Methods A total of 96 cases ( 102 eyes) with PACG were divided into two groups.The patients in observation group ( 56 cases,59 eyes) were treated with cataract phacoemulsification combined with goniosynechialysis,and the control group ( 40 cases,43 eyes) with trabeculectomy.The intraocular pressure,visual acuity,and angles’shape of two groups were compared after the surgery.Results The postoperative intraocular pressure ( IOP) of observation group was ( 14.5±3.0) mmHg( 1 kPa = 7.5 mmHg) ,the control group was ( 16.7±2.7) mmHg,there were significant differences compared with preoperation( all P 0.01) ,but there was no significant difference between two groups( P 0.05) .The postoperative visual acuity of observation group and control group were 0.45±0.20 and 0.26±0.16,there were significant differences compared with pre-operation( P 0.01 or 0.05) ,and significant difference between two groups( P 0.05) .The postoperative central chamber thickness ( CCT) of observation group and control group were ( 3.20±0.49) mm and ( 1.96±0.39) mm,respectively,the postoperative corneal limbus thickness were 0.403 CCT and 0.290 CCT,there were statistical differences( P 0.01 or 0.05) ,but the improvement of the observation group was significantly better than the control group ( all P 0.01) .All patients' chamber angular in observation group opened larger than 180°,but in control group the rate was 72.1%( 31 eyes) ,the difference was significant( P 0.01) .Conclusion Phacoemulsification combined with goniosynechialysis is effective for PACG,which has better effect on IOP control and visual acuity recovery.
