Clinical analysis of ARDS induced by severely acute pancreatitis

Objective To analyze the clinical features of ARDS induced by severe acute pancreatitis (SAP) and the effects of reasonable respiratory support at the early stage.Methods The clinical data of 84 SAP patients were studied retrospectively.Multiple arterial blood gas and chest-lung X-ray were examined within 24 hours of the onset of SAP. 54 patients with ARDS were treated with lower tidal volumes under respiratory machine and lung-opened tactics.Results The ratio of SAP combined with ARDS was 61.90%.The possibility of combined ARDS when PaO_28.0 kPa excessed 60%,and when PaO_24.3 kPa there would be 90% of the patients occurred ARDS.Acid-base disorders were very common.Approximately 50% of the patients appeared chest-lung abnormalities by X-ray examination.The ARDS cases accepted mechanical ventilation treatment for (13～172.5) h,their arterial blood gas improved obviously, among them 40 cases were cured, 12 cases died, death rate was 30%.Conclusion The respiratory function of SAP patients should be carefully observed so that ARDS cases could be found at the early stage.Mechanical ventilation show beneficial effects on the ARDS treatment.
