Abstract
[Objective]To explore the blood pressure changes and clinical features of patients over 70 years old,who are suffering from hypertension complicated with progressive cerebral infarction.[Methods]36 essential hypertension patients below 70 years old in Xinglong People's Hospital of Chengde City from February 2012 to 2013August were collected as the control group,while 33 patients with hypertension complicated with progressive cerebral infarction,who were over 70 years old,were chosen as the study group.Their blood pressure levels were monitored and recorded by the ambulatory blood pressure monitoring(ABPM),to analyze the clinical features of blood pressure,namely the circadian rhythm.[Results]The 24 hours pulse pressure,the daytime pulse pressure and the night pulse pressure in the study group was respectively(50.7 ± 5.2) mmHg,(50.3 ± 6.3) mmHg and(51.9 ±6.2) mmHg,which was significantly higher than that in the control group [(45.0 ±5.8) mmHg,(46.6 ±3.3) mmHg and(44.1 ± 3.8) mmHg].24 hours systolic pressure,the daytime systolic pressure and the night systolic pressure in the study group was respectively(124.9 ± 10.2) mmHg and(120.2 ± 4.2) mmHg and(124.9 ± 10.2) mmHg,which was significantly higher than that in the control group [(118.2 ± 11.7) mmHg,(118.9 ± 9.8) mmHg and(115.2 ± 11.3) mmHg].The clinical circadian change of blood pressure showed:there were 3 cases of dipper-shaped blood pressure in the study group,accounting for9.09% of the total cases,while there were 15 cases of dipper-shaped blood pressure in the control group,accounting for 41.67%,and the difference was statistically significant(χ2= 9.48,P = 0.008 2 0.05).[Conclusion]The patients over 70 years old,who are suffering from hypertension complicated with progressive cerebral infarction,have appeared the elevated blood pressure and pulse pressure in night,which is easy to induce the cerebral infarction in elderly hypertension patients.Therefore,it is necessary to strengthen the monitoring of patients in night,to prevent the accidents.
