Abstract
Objective:The value of cytology and high-risk human pappilomavirus(HR-HPV)testing in screening cervical precancerous disease was assessed.Methods:813 women with abnormal cervical smear were screened simultaneously using HPV DNA testing by hybrid captureⅡ(HC-Ⅱ)and biopsy under the colposcopy.We compared the test performance of cytology and HR-HPV DNA testing with final pathological diagnosis.Results:Among 813 cases,the pathological diagnosis as NILM、CINⅠ、CINⅡ~Ⅲ were 228,522 and 63 cases respectively.Correspondingly,the positive rates of HR-HPV were 9.6 %(22/228),52.5%(274/522)and 93.7 %(59/63).For the detection of CIN,the sensitivity of cytology,HR-HPV,cytology combined with HR-HPV were 46.7%、56.9%、66.5 %,respectively and the specificity were 96.9%、90.4%、89.0% respectively.For the detection of CINⅡ~Ⅲ,the sensitivity of cytology,HR-HPV,cytology combined with HR-HPV were 93.7%,93.7%,100% and the specificity were 77.2%,60.5%,53.2%,the negative predictive value were 99.1%,99.2%,100% respectively.Conclusions:Cytology and HR-HPV testing are efficient methods for screening cervical precancerous diseases.Cytology combined with HR-HPV testing can increase the sensitivity and negative predictive value,but it does not increase the specificity.
