症例は51歳，男性．直腸癌に対し砕石位での腹腔鏡下手術が施行された．抜管後より左下肢の痛みを訴え，術翌日コンパートメント症候群の診断で，緊急筋膜切開を必要とした．以後，侵害受容性疼痛と神経障害性疼痛に対する症状緩和に難渋したが，4ヵ月後に自力歩行可能となり退院した．長時間に及ぶ砕石位での手術の際にはコンパートメント症候群を念頭においた管理が必要である．
