失神は診察時その病態が消失している事が多く,原因診断が難しい.しかし心原性失神は予後不良のため,見逃さないように注意して病歴をとる.安静時心電図が正常でも,しばらく心電図モニターをしてみるとよい.心原性の証拠が乏しくても直ちに入院の必要があるものは,最近頻回の失神,心機能低下,心室頻拍が疑われる時である.
