발작성 심실상성 빈맥(paroxysmal supraventricular tachycardia, PSVT)은 회귀성 빈맥(reentrant tachycardia)을 나타내는 부정맥군을 말하며 방실결절 회귀성 빈맥(atrioventricular nodal reentrant tachycardia, AVNRT)과 부회로를 통한 방실 회귀성 빈맥(atrioventricular reentrant tachycardia, AVRT)이 여 기에 해당된다.1,2) PSVT의 일반적인 심전도상 진단기준은 빈맥이 갑작스럽게 발생하며 정상적인 QRS파 혹은 기존 각 차단(bundle branch block)이 있으면서 RR 간격이 40 ms 이 하이고 심실 심박수가 분당 120회 이상이며 QRS 파를 앞서 는 P파가 없을 경우이다.2-5) 증상이 없는 경우도 있지만 급 격한 두근거림, 어지러움, 흉부의 조임, 호흡곤란 등이 나타 날 수 있고, 심한 경우 빈맥으로 인한 심실로의 용적 충만 소실로 저혈압, 의식소실과 좌심방압의 상승으로 인한 급성 폐부종 등이 동반될 수 있다.6,7) 마취와 연관하여 발생하는 전체 부정맥 중 PSVT는 약 2.5% 정도 차지한다고 알려져 있으며 이는 마취 중 자율신경계의 변화, 약물효과, 정맥 내 용적의 변화 등에 의해 유발될 수 있다.8)
