関節は無菌状態にある．感染性関節炎は関節の損傷あるいは血行性など関節外より病原微生物が直接的に関節部に進入して生じる．細菌性関節炎は急速に進行し関節破壊を生じるので早急な対応を必須とする．真菌，マイコプラズマ，ウイルス，スピロヘータ等いずれでも関節炎は起こしうる．特に老齢やステロイド，免疫抑制薬使用状態を含む免疫不全状態の例では本疾患に留意する必要がある．
