[Microsurgery and infertility: present and future prospects].

Abstract

The use of microsurgery to reverse infertility in men and women is discussed. Microsurgery has been shown to have a success rate of 95-100% in anastomosis of the vas deferens compared with 60-83% with traditional surgery. The anastomosis of the Fallopian tubes has had an approximate 70% success rate with microsurgery compared with 39-47% with regular surgery. Tubouterine anastomosis has shown a 53-60% frequency of success with microsurgery compared with 43% with traditional surgery. Microsurgery has probable applications for the treatment of varicoceles or the performance of epididymo-deferential anastomosis in men, and for the performance of tubouterine implantation,neosalpingostomies, fibrioplasties, and salpingolysis. Rare cases of auto- and heterotesticular transplantation, implantation of the epigastric artery, and reanastomosis of an amputated penis using microsurgery have been reported. The use of microsurgery will open many new fields in treating human infertility, e.g., testicular, ovarian, and uteral transplants.
